Payment level in C&P not getting populated in Salesforce Event Management

I am planning an event with multiple ticket levels. The last level, "single ticket"'s amount is not getting over to Salesforce. It is showing as $0 when it should be showing as $118.

It is there because I added it manually. However, it is not showing up on the payment form (see attached.). Also, can you help me understand how I am not longer seeing the payment form that I setup in C&P (had blue and red coloring and nice formatting).

Sorry but I am bit confused. The form in the above image is NOT Events form.

C&P Events site:

As you can see the above Event list the Level you are looking for. However, may I know where exactly you are expecting to see the Events form and levels?
