Please read the items below before registering. You will be asked to sign a waiver in regards to these matters at the end of the registration process. PHOTOGRAPH/VIDEO/AUDIO/MEDIA CONSENT & RELEASE FORM FOR MINOR (UNDER 18 YEARS OF AGE) I, hereby consent to and authorize the Roman Catholic Diocese of Lafayette, Louisiana, (the Diocese) and all entities, representatives, employees, and agents operating under its authority to record, use, edit, reproduce, and/or publish photographs, video, audio, and/or other media that may portray and/or relate to the aforementioned minor child, his/her image, likeness and/or voice, without compensation. I understand that these materials may be used in various print and electronic media, including but not limited to the Diocesan website and the Diocesan publication, Acadiana Catholic, and/or for other endeavors related to Diocesan interests. I understand that the Diocese may use and/or publish materials relating to the aforementioned minor child and/or use his/her photograph, voice, video images, and other media relating to said minor child in any manner that the Diocese deems appropriate in order to promote and/or publicize its programs, or for any other lawful purpose. This authorization shall not expire and will remain effective indefinitely until rescinded in writing. By signing electronically above I am verifying that I have read the above statements. Mandatory Life Teen Registration Our High School religious education program is called Life Teen. Life Teen attendance is mandatory for all students who do not attend a Catholic School and who wish to make their Confirmation. Please note that as per our new update Country is required for State picklist and in your case, the country is not required field which is the reason state options not display:

Please make it required and it will display the state picklist.

Okay. Even with the country and state checked in both boxes on the form country isn't displayed and state still isn't operable. Please help!

It was not checked. However, we have checked those as required and it is working fine now.

I'm not sure if I'm losing my mind or if it's a browser issue or what, but it's still not working for this field. I've taken screenshots to prove that the necessary items are checked. What am I missing? Please help.

Hello. Any chance you are able to look at my last problem? The other two were fixed. I'm not sure why I can't fix them on my end. I'm on the support line now but no C and P representative is coming on. Others are on the line too saying they had this experience yesterday with no one showing up. Hoping to figure out why I'm having issues with the "state" field, as seen in my post above.

I figured out the issue. I was on the registrant tab and not the tab i needed to be on. I fixed the issue. Thank you!
