I understand that Invoice has been removed as a standard payment type in the new release. How do you set up a custom payment type of invoice so that the opportunities don't show up as paid?

The ACH has not been removed. Please go to the EVENT > DEFAULT SETTING for the box office and see if you have it checked.
We do not have access to your account so we cannot check your instance. If you grant us access we can check.
and we are not sure what you mean by "header has disappeared". Please elaborate.
You have also stated that you are up-to-date but the instance we checked seems to be not up-to-date. Please let us know the Salesforce Org ID you are checking.Regards,
