Custom Questions are Repeating on Registration Page

We have custom questions set up for the various registration levels we have for our event, but for some reason the questions are showing up multiple times on each registration level.

When I go to view the registration level though each question is only listed once under the "Event Custom Fields" section.

I am not sure what could be causing this.

Also, this may be related...

I went in to edit some old registration levels from previous events in case those were part of the problem and when I clicked to edit one I got this error:

List has no rows for assignment to SObject
An unexpected error has occurred. Your solution provider has been notified. (CnP_PaaS_EVT)

The following summarizes what we have done to your account:

• Reviewed your account and all installed versions Event 2.3009 and PaaS 8.2000
• Reviewed your post-installation and realized that it was not completed. We completed the post-installation of release 2.x
• Once Event 2.x is installed, Event 1.x events can no longer be used and need to be modified.

What you need to do next:

• Go to the "Event Management Tab"

• You may change the design to a fully customizable page using the new Designer and the HTML editor. We also have a number of templates that may help.
• You need to review all your custom questions and make sure all your custom questions are assigned to the correct level. In converting event 1.x to 2.x events the custom questions are redesigned to make it easier to apply the same question to multiple levels.
