State as a required field doesn't show up?

Hi,
Please take a look at this record that's newly created https://na35.salesforce.com/0014100001lbtUUAAY . You can see that under "State" it shows "Select", which comes from the donation form https://urm.org/n/donate-8/. It is a required field. So shouldn't a state value required to be picked and it shouldn't allow "Select" as an option?

This is the first time I've seen this happened even though our https://urm.org/n/donate-8/ form has been used since last year. Please help. Access has been granted.

One of our developers reviewed the FaaS donate page and concluded that you will need to replace the following element: <option>-- Select --</option> TO <option value="">-- Select --</option>
