The reason you are receiving the above error is because you haven't re-establish the connection after upgrading to latest version. The new update you have done has additional fields and it is recommended that you re-establish connection at your earliest convenience. Once connection has been established you should see the following fields in the C&P Settings > Account in Salesforce to be updated. See below:

It is best to connect one of the accounts with DIRECT connection and the others as SHARED connection.
