Foreign payments are duplicated

We have been getting a rash of "stutter payments", mostly from foreign sites. The pattern is that the customer will have a succession of failed payment (5001 error codes), and when a payment finally goes through, it posts twice or four times.

I have attached some recent examples.

What can we do to prevent these?
