In the current release we don't support checkboxes in the field matching. TEXT, LONG TEXT, and PICK LIST are the only supported form types.

No, I'm not using the E-newsletter subscription checkbox in the checkout page (didn't know there was one). I have created a CUSTOM QUESTION that asks "Do you want to receive our e-newsletter?" The person has to type "yes" or "no" or anything else (one typed "sure"). All I want to do is map the "no" text response to the standard Salesforce E-mail Opt-Out checkbox on the Salesforce Contact page. My problem is that in the custom mapping tab (your screenshot) C&P does not show this Salesforce field in the drop-down under Field Map. Since it's a standard SF field, why isn't it in the Field Map list? It's not even a custom field.

I see now what you are referring to. You are referring to the eNewsletter subscription checkbox in the checkout pages.
Those are not posted to Salesforce and are not available.
You may want to add a custom question to the form and the answer then may be mapped. The mapping is ONLY for custom questions and not any other fields.

Yes, I have the latest version installed and I have used this screen to map my other question. The problem with the e-mail opt-out question is that in the Field Map, I don't see the E-mail Opt-Out choice. Even if I did, how would I indicate that if the answer is "no" that the box should be checked, and if "yes" the box should not be checked? Currently I have no field to map a "yes" answer to, but I would like to map the "no" answer to the standard Salesforce E-mail Opt-Out.

What version of the payment application do you have installed?
If you are using the latest release, have you tried using the Custom Question mapping?
You can add the question and the answer to the appropriate fields.

I have two custom questions. One of them is an e-mail question: would they like to receive our newsletter. I tried mapping it to the e-mail Opt-Out field if someone types "no" but C&P doesn't seem to give me that field as an option when mapping custom questions. Even if it did, I'm not sure how to map a "No" to equal a check in the E-mail Opt-Out checkbox.
Then I thought I could do a report to tell me who typed "No" to make sure I catch them and don't send them e-mail. However, when I tried to create a report from the C&P reports, I did find a field called "CnP Order Number: Custom Question(s)" but that doesn't populate with anything.
Please tell me how to map to the SF E-mail Opt-Out field, or how to create a report so I can see the answer to all my custom questions per Contact Name/ID.
