We have spent a lot of time on this issue. I have also spent a lot of time with this very issue researching and understanding the challenges.
To answer your question regarding the accessibility - the answer is YES. Our forms are tested against several screen readers. There is also an option in the Campaign setting that allows you to enable a feature that further helps with screen reader. The feature that causes an issue is the Calendar used for future date transaction. If you wish to be 100% compatible with screen readers please enable pull down menus for the calendar - if you want to enable future date transaction.
The visual calendars are not screen reader compatible so we replace them with pull down menus for Day, Month, and Year.
Our testing was primarily done with https://www.nvaccess.org/ -- we also considered Jaws but settled on the NVAccess as we found it to be complete.
We don't have any statements regarding this as we don't think one is needed. All forms have to be readable by screen readers and not sure why one has to attest to it. It is like attesting that we have used English words - I am not trivializing the question, I am just stating that this is standard protocol and has to be part of all forms.
