Form background image won't upload

I'm trying to upload a background image for a form, but the file won't upload. I've tried .jpg, .png, and .tif, and none will work. I've also tried reducing the file size. I keep getting this error:


When I click ok, I'm not getting any additional information as to why it's not working.

Any idea why I'm unable to upload the file?
