Form Formatting Issues

In an attempt to clean up a Connect form I've collapsed the page header, removed the transaction total on the top, and ensured there is no left over text in the page header, but I can't get rid of a small white line that remains at the top of the form as well as a thicker border, as if a header is not fully removed. Another form in the same Connect account, but a different Connect campaign, does not have this line. See the two attached pictures for comparison. Any ideas as to what needs to be done to remove this. C&P Account 32905, default campaign, only form in campaign

We reviewed and found that you have not removed the complete page header which was causing the issue. However, we have fixed it.
