Add Additional Fee To Giving Page

I am trying to add the additional fee option, where a donor can pay the card fees in their donation. I have gone through and added the appropriate information to the campaign, but it is not showing up on the giving form associated with the campaign. Here is the link to the giving page:

Please note that when initially page loads, the amount is $0.00, in this case, it won't show the additional fee options. You will need to select the amount and based on the amount it will calculate the additional fee and display on the page.

I am having the same difficulty adding the "Additional Fee" section to my payment form. I've updated the Campaign details, but have not been able to get the Additional Fee question to show up on the payment form.

Have you clicked on any of the amount buttons and tested?
