Canadian specific changes to form

Good evening,

I am working with a Canadian client that requires:
1. Addition of a middle initial/name (the Canada Revenue Agency requires a middle initial on charitable receipts)
2. Change the state picklist to either province picklist or text field

From reading previous posts, I saw that middle initial/name can't be added to the contact info. Is a custom question the only way to capture middle name?

What steps are needed to remove the state picklist values?

As of now, yes the only way to capture middle name is custom questions.

Regarding the state picklist, it is NOT possible to remove that functionality. The country which have states will show in the picklist whereas those who doesn't have states offers Text box.

Sorry, you can't set it default as the Country selection is based on patrons IP address. Please note that in Connect forms you can't make the fields optional, those all fields are mandatory due to security reasons.
