I have encountered this twice now (in two different orgs). The campaign in Connect is set up with the additional fee "Enabled - Opt-In," but the form is not showing the additional fee section. I've tried clearing my internet cache, and disabling the additional fee and updating the form, then re-enabling the fee and saving the form again. All with no luck.
