One form displays well on responsive mobile, another does not ?!?

I've got two forms for our Funddrive that display well on a desktop, but on a mobile device - One Time donation form doesn't display properly on a smartphone, yet Monthly donations do.

Any idea what could cause one to display and another not? We just kicked off funddrive, so any leads would be very appreciated.

Actually, I can't click on any of the $$ buttons to actually work through the process on mobile. HELP!
