Creating custom question checkbox while hiding question text

I'm trying to create a few easy optional check-box custom questions for a check-out form- "I would like to remain anonymous." and "This is a payment toward a pledge." Ideally, I would like to hide the Custom Question text (while still naming it so that I can map the values to Salesforce) and have only the single check box option visible. Is that possible?
