Does anyone know how we can create a prompt that pops up during checkout and asks the donor if they want to convert their one time gift into a recurring gift? I've encountered this on other websites (see attached photo). Thanks, in advance.

Ideally the second link would be the place (instead of the square box that prompts for "one-time" or "payment plan").

These classic forms do not have that option and you cannot add Javascript to these pages. In 2016 these forms will retire and all will be moved to Connect and widgets. Have you explored those forms? These forms are designed to be added to your site but they do not offer what you are asking for. In Connect forms you may remove the option of one time payment and make it all recurring.
If you wish to have a complete customization, we recommend exploring our API but that requires knowledge of HTML programming. Here is info about our API: http://manual.clickandpledge.com/Form-as-a-Service.html
