Donor from Brazil repeated donation 104 times... user error or system error?

Hi,

Last night, a donor from Brazil somehow made 104 of the same $25 donation. Her email address is not valid, so I am unable to email, and will be trying to contact her by phone. I just wanted to check in and see if you knew of any system errors that could explain this, or if it was user error?
