How do I create an online FaaS donation page with WordPress?

I’ve created a form in WordPress but I don’t think it is linked to anything in Click & Pledge. How do I link them? Also, what information is needed in the form for it to link with click & pledge?
Any information about starting FaaS with WordPress would be appreciated.

The form fields need to be named according to the convention shown in the example. Also the manual has full description of each form field:

FaaS forms are quite easy and many of our clients use them in WordPress.
