Error Received: Last operation caused fatal error

Hello, I am posting in regards to what appears to possibly be two different error types.

After working with a third-party (Swapps) to build a new FaaS form, we attempted to submit a donation using the new monthly gift option that was available and received the error message "It appears that your last operation has caused a fatal error in our system...."

Additionally, the one-time gift option produces the message "Error" in the space of our form (that is without the redirect to the fatal error message) and we are not sure how these are different.

These forms are not currently live on our website.
