How do I get my form to work with your system?

I've created a form in Dreamweaver CS5 for a client (has an account with C&P) and I'm wondering what I need to do to get it to work with your system. I am more of a designer than a programmer so although I have a basic understanding of html, I'm pretty clueless when it comes to forms. The form itself is complete...now what?

Points to pay attention to regarding your form:

• State & Country: These pull down menus are provided in our manual. When you post these values to us you need to send it according to the codes we provide. So simply copy the country & state as we have provided. You may want to use them as server side include in DM. • Giving option: Adding the credit card fee is not allowed by credit card companies. If you wish to add it then you need some sort of JavaScript to calculate the fee dynamically according to the amount of donation selected or entered as free text.
• Total Donation: That field needs to have a JS to calculate the fee and the donation entered.

Other than the above simply change the form field names according to our naming convention and post it. It is easy.
