I will go ahead and update the version.
However, the issue is that our SKU for that form is NOT the same as the campaign.
SKU=ntk-donate
campaign=WQ
is it not possible to have seperate and distinct SKU and primary campaign source?

In reviewing your post we observed the following:
Point 1:
Your version of GF plugin is old and we recommend that you upgrade to the latest version. The latest version number is 3 and your release is as shown below:
Point 2:
Per the following we are a bit confused (more than a bit).
Why are these fields being passed as custom questions? OnSuccessURL, etc. are all FaaS form fields for API. They have nothing to do with the GravityForm. Please review: http://manual.clickandpledge.com/Gra..._for_a_Product
All these fields need to be removed as they serve no purpose and as they are custom fields they will show up on the receipt. This is a bit confusing as to why they show up there.
Point 3:
The campaign is being passed as a custom question. While this may work if you do "Custom Question Mapping" in the settings, it is not really the optimal way of doing it. We suggest you use the SKU and map the SKU to a campaign in the C&P Settings.
Point 4:
In the product definition you have used the SKU = ntk-donate
You may use this and map this SKU to a primary campaign in the C&P Settings. You have done similar mapping but have not done it for this SKU
If you may the SKU in the "Primary Campaign Source" section you will be able to easily map it to a campaign.

Good day!
No problem we will investigate further. For future we strongly recommend that you use the Maintenance (C&P Settings > Maintenance) for deleting any transactions. Using the DELETE feature of Maintenance you will be able to remove all traces of the transaction instead of its parts.
We will update this once we review the issue further.

This is a bit strange. Please see:
The Opportunity does not exist. Since we do not see any errors in the C&P Data we are confident that the transaction was created successfully.
Any idea what has happened to the Opportunity?

All transactions have an order number. You may find them in the C&P Transaction.
also for the transaction.

We need to see a transaction that is not doing as expected. This naturally is a matter of your settings and we need to review the C&P Data and compare to your settings.

Campaigns
We are having some trouble with Gravity Forms. We were able to get the SKU to work correctly but can't seem to be able to get Primary Campaign Source to transfer to Salesforce properly. It shows up in the transaction record but does not populate on the donation page.
we have tried every variation recommended here: http://manual.clickandpledge.com/Gravity-Forms.html and are totally out of ideas.
