Is there any way to capture the donor's address/city/state/zip?
I've gone into the smsgiving- form under C&P MyConnect and see I have the option of making email optional or mandatory, but no address?

NO, through SMSgiving you can only collect the email. We designed the SMSgiving to make it easier for people to use the feature for giving. We simply created something that is to the point and only does giving.
