Connect 2.1: Cloning of forms, payment button alignment

New Features:

1. Forms may be saved as a new name and with a new campaign. The option provides an easy method for copying an existing form in the same campaign or another campaign.
2. Payment buttons may now be set to show in 1, 2, 3, or 4 in a row. Using 1 button will result in vertical alignment since only one button will show in a single row.

Bug Fixes:

1. Default recurring periodicity was not showing correctly in certain configurations.
